Please ensure you fill out the entire form and leave an up to date contact number. 
This is to help ensure your medication is prepared in a timely manner.		    

Medication requests submitted Monday – Thursday will be ready to collect within 48 hours.

Requests submitted Friday – Sunday will be ready to collect the following Tuesday onwards.

(This service may take longer during bank holidays).
